Job Summary


Neighborhood House has a longstanding commitment to address social determinants of health and decrease health disparities in King County. The primary role of this position is overseeing the team of Community Nurses who provide clinical leadership, medical expertise, training, and consultation to staff and clients at Neighborhood House. The Community Health Nurse Manager reports to the Director of Community Health with key support from the Family and Health Services Director . The programs at Neighborhood House assist a large and diverse community-dwelling client population with multiple risk factors including housing instability, physical and/or behavioral health problems, long-term care needs, substance use disorders cognitive status concerns, and physical or emotional neglect or abuse. The Community Health Nurse Manager will be a dynamic member of the agency's leadership team, overseeing the work of the Community Health Nurses and developing programs that will decrease health disparities and address the health barriers facing our clients. The ideal candidate will be a Registered Nurse with experience supervising staff, leading community-based health initiatives, navigating within a dynamic and flexible atmosphere, and working with high-acuity populations. This position will be primarily located in Seattle, Tukwila, and South King County.


Responsibilities


Program Design and Operation (50%)

  • Work directly with agency staff and community members to envision, design, implement, and evaluate initiatives that will address health disparities experienced by the communities Neighborhood House serves.
  • With the nursing team, plan and perform onsite education and training on disease management and health promotion with staff, clients, and client caregivers.
  • Design, implement, and evaluate workflows to ensure that the nursing team meets the complex funder requirements and follows federal, state, and program policies and procedures for nursing referrals for a variety of risk factors.
  • Discuss medical/nursing issues and responsibilities with the program leadership on a regular basis.
  • Perform other duties as assigned.
Supervision and Administration (50%)
  • Recruit, hire, train, and supervise a qualified team of Community Health Nurses; ensure program nursing requirements are met to the agency and programs' audit standards.
  • Support Nursing staff in providing nursing services as needed; this may include meeting with staff or clients for consultation, assessment, and/or intervention.
  • Provide clear expectations, set individual and team goals, provide regular formal and informal feedback on ongoing performance
  • Assess staff development needs and provide appropriate on-going professional training appropriate to the field.
  • Participate in grant writing and fundraising activities as requested.
  • Manage all program elements and participant activities to meet the highest standards of quality and to meet all funder and internal performance outcomes.
  • Develop and maintain key project documents to support all aspects of programming, including regular program reports, evaluations, satisfaction surveys, etc.
  • Participate in NH leadership teams and/or workgroups as necessary.
  • Represent NH by participating in activities or professional networks or other related networks as assigned.
  • Perform other duties as assigned.
Qualifications


Minimum Qualifications
  • A Bachelor of Science in Nursing.
  • Two years of clinical nursing experience with children, families, low-income, elderly, and/or disabled adults; OR three to four years of clinical nursing experience with any age group.
  • Current Washington State Registered Nurse license; maintain license throughout employment.
  • 1-2 years of experience in a leadership role.
  • Experience coordinating care plans with social services, physicians, home health agencies, hospitals, family caregivers and health industry providers for clients with complex medical conditions.
  • Demonstrated ability to thrive in a culturally diverse workplace dedicated to equity and inclusion.

  • High degree of medical knowledge including experience developing and educating patients with self-care tools and procedures.
  • Demonstrated ability to manage and organize multiple tasks and meet deadlines in a fast-paced environment while maintaining attention to accuracy.
  • Ability to perform job duties within the context of complex Federal and State regulations and to complete extensive technical documentation.
  • Demonstrated ability to communicate effectively, both orally and in writing, with people from a wide range of cultural and economic backgrounds.
  • Strong interpersonal skills and proven ability as a team player.
  • Proficiency in MS Office suite.
  • Ability to make on-site visits to clients and attend meetings at community-based agencies and providers.
  • Must have reliable independent transportation for frequent travel to client homes and service sites.
  • This position is expected to work at a Neighborhood House location in person 2-3 days a week. In addition, the position will require visits to clients' homes or early childhood education sites
Desired Qualifications
  • 2+ years supervising nursing professionals.
  • Skill and experience with computerized CARE assessment program and/or another client management or medical record databases such as Collective Medical or PRISM.
  • Experience working in Early Head Start, Head Start, or other child development programs.
  • Experience working with adults who are elderly and/or adults with disabilities.
  • Experience working directly with unhoused individuals, immigrant, and/or refugee populations.
  • Second language fluency, with ability to effectively communicate orally and in writing in English and second language.
  • Home-visiting and/or community-based nursing experience.
Required Credentials
  • Registered Nurse in Washington State
  • Must pass Neighborhood House background check requirements.
  • Position requires Washington State bi-annual criminal background re-check.
Post-Hire on-the-job Training Requirements
  • Current First Aid Certificate within 30 days of hire.
  • Current CPR Certificate within 30 days of hire.
  • Sexual Harassment Training within 30 days of hire.
  • Child Abuse and Neglect training within 2 weeks of hire.
  • Mental Health First Aide training within 60 days of hire.
