Duties include: * Develop and updateindividualized client care plans * Conductsupervisory RN visitsto assess condition and quality of services * Providehands-on caregiver training * Ensure caregivers demonstrate competency with delegated tasks * Reassess each client's needs and satisfaction every 90 days or as needed * Document any changes in client condition and adjust care plans accordingly * Maintain communication with clients and designated family member * Reports to the Agency Director any condition which requires further attention in providing home care services Why JoinGriswold?
