Centra Healthcare is seeking a LPN / LVN Long Term Care for a travel job in Poughkeepsie, New York.

Job Description & Requirements

Job Duties & Responsibilities

• Direct care and support for adults with developmental disabilities
• Assisting with ADLs (Activities of Daily Living):
  • Bathing
  • Dressing
  • Toileting/incontinence care
  • Feeding/meal support
  • Ambulation or transferring support
• Hygiene and cleaning (supporting personal care and sanitary living environment)
• G-tube care (gastrostomy tube)
• Ostomy care (colostomy/ileostomy)
• Medication administration as prescribed by physicians
• Monitoring health status and reporting changes
• Collaborating with RNs and other healthcare professionals
• Supporting a safe, inclusive, and respectful living environment

Experience & Requirements

• Active New York LPN license (unrestricted)
• Prior experience with:
  • Developmentally disabled adults (preferred)
  • G-tube and ostomy care
  • Behavioral or residential care a plus
• Strong understanding of:
  • Person-centered care
  • De-escalation techniques and compassionate communication
• Ability to work in team-based, home-like settings
• CPR/BLS certification (required or willing to obtain)

Schedule

• Shifts vary by residence (often include early morning, evening, and possibly overnight)
• Weekday and weekend coverage may be needed
